If you have the paper bill, the charges should all be lumped down at the bottom for each person. We had a similar but different problem with my sisters phone, she went to a site to get a free ringtone and entered her phone number but then didn't complete the other info because it wasn't free, it was "free". So all they need is the number. You should be able to look into it with your provider, they might be able to get you your money back.
